When you’ve been in the hospital due to illness, injury or a medical procedure and you’re ready for discharge but not quite prepared to return home, Methodist Senior Services’ Bridge to Home program offers the post-acute, short-term inpatient rehabilitation services to help you get back to your life.
Through the MSS Senior Tx care program and our partnership with Vertis Therapy, we offer short-term rehabilitation at four communities. Our rehabilitation team is skilled in a comprehensive approach that includes evaluation, treatment and ongoing assessment of therapy plans to achieve individualized therapy goals tailored to each person’s unique needs.
Bridge to Home services provide access to 24-hour nursing care authorized by a physician, along with rehabilitative care including physical, occupational and speech therapy. All MSS campuses offering Bridge to Home and skilled nursing services are Medicare certified.
